THE CORONARY HEART DISEASE TEAM
The Integrated Coronary Heart Disease Team consists of:
Dr Alun Jones – GP with Special Interest in Cardiology Integrated Care Partnership
Sister Suzy Williams – CHD Nurse for Integrated Care Partnership
Within the ICP Heart Disease Team we aim to look after all aspects of heart disease from prevention to reducing risk of further events such as heart attacks ( MI ) and strokes to all those patients with existing coronary artery disease.
REGULAR REVIEW CLINIC
Every nine months all patients with coronary heart disease will be offered an appointment for a review with our CHD nurse. During this appointment a review of symptoms and advice will be given to patients to reduce further their overall risk. Any concerns regarding your cardiac health will be addressed at this appointment. We will assess risk factors including weight, cholesterol, blood pressure, smoking status, diet and exercise.
British Heart Foundation booklets and a bimonthly magazine are available. We always try and involve patients in trying to achieve a healthy heart.
FOLLOW UP FROM CHD CLINIC
If, following a review from the CHD clinic, it is apparent that further assessment is required an appointment will be made with the in house CHD doctor – Dr A Jones or the Consultant Cardiologist .
If appropriate you will be referred for the following:
- ECG recording
- 24 hour blood pressure recording
- 24 hour ECG recording
For 24hr blood pressure and 24hr ECG recordings patients will be telephoned to arrange fitting which will be done at the Old Cottage Hospital. Patients are then required to re-attend the following day for the equipment to be removed and the recording analyzed.
Following these investigations patients will either be contacted by their own doctor or offered an appointment at the Cardiac Clinic.
Patients who require other cardiac investigations will be offered an appointment for this at St George’s Hospital Tooting.
Investigations available include:
- Heart Ultrasound (echocardiogram)
- Angiogram /angioplasty
- Exercise testing
- Holter monitor
The average waiting time for the above investigations depend on the clinical urgency. However, as a guide a routine appointment is approximately 8 weeks.
