QUALITY AND OUTCOMES FRAMEWORK (QOF) ASSESSMENT 2007-2008

QOF was instigated by the government in 2004 as an annual measure of performance for general practice.
We are proud that this achievement reflects the high standards of care that we strive to provide for our patients.

HEALTHCARE TRAVEL COSTS SCHEME
The Healthcare Travel Costs Scheme ( HTCS ) was set up in 1988, as part of the NHS Low Income Scheme, to provide financial assistance to those patients who do not have a medical need for ambulance transport, but who require assistance with their travel costs.
Under the scheme, patients on low incomes or receiving specific qualifying benefits or allowances are reimbursed in part or in full for costs incurred in travelling to receive certain NHS services, where their journey meets certain criteria.
1. The patient must be: a) in receipt of one of the qualifying benefits or allowances specified in the 2003 Regulations( or in certain cases be a member of the same family as a person receiving a qualifying benefit or allowance ) or
b) be named on an NHS Low Income Scheme certificate HC2 or HC3 ( or in certain cases be a member of the same family as a person named on an NHS Low Income Scheme certificate )
2. The journey undertaken must be made to receive services under the National Health Service Act 2006, which are not primary medical or primary dental care services, for which the patient has been referred by a doctor or dentist
3. Where a doctor or dentist has provided the primary medical or primary dental services which lead to the referral for non - primary care services, those services must not be provided on the same visit and in the same premises as the primary medical or primary dental services which lead to that referral.
HTCS does not apply to people visiting others in hospital, or visiting a GP or Dentist for routine primary medical or dental care - for example, for regular check - ups, vaccinations or screening appointments. Urgent primary care appointments during the out of hours period are also excluded.

RETIREMENT
Dr. Richard Stott retired at the end of March this year after twenty five years of service to patients in Epsom from surgeries at 52 The High Street, Cedars Cottage in Church Street and The Old Cottage Hospital Surgery. Richard has maintained the highest standards in general practice, setting up Baby & Child Health clinics, as well as training new generations of GPs. We shall miss his enthusiastic and intellectual presence, but we are pleased he will be enjoying furthering his artistic skills and lowering his golf handicap.
ICP is pleased to welcome Dr. Serene Bhatia who joins us from April with a responsibility to Dr. Stott's former patients.
Dr. Bhatia trained at University College London and has worked in Paediatrics, A & E and Obstetrics & Gynaecology at Epsom General Hospital. She has completed a year's training in GP practice in Tadworth. Dr. Bhatia holds MBBS and Membership of the Royal College of General Practitioners qualifications.

Dr. Jyoti Patel MBBS, BSc, DRCOG, DCH
Trained at Barts and The London. Qualified 2004. BSc in Diabetes and Endocrinology at Kings College London - 2003. Worked at Epsom General Hospital for 1.5 yrs rotating through A& E, Obstetrics & Gynaecology and Paediatrics before joining ICP.
Dr. Jeyarani Arunagiriraj MBBS
Undergraduate training in India, at the University of Madras. Qualified in 1985. S.H.O. posts were in Scotland and worked for Dumfries & Galloway NHS Trust before joining ICP. Interested in Family Planning & Sexual Health as well as Womens' Health.
BEN NEVIS CHALLENGE - 17th/18th MAY
Following the success on Snowdon last year, our intrepid team are gearing up to climb Ben Nevis in May. Those who have been talked into taking part are;

FUTURE PLANS
We are proposing to introduce :
A text messaging service -
By handing in your mobile number you will be eligible to receive via text message, relevant health information from your GP e.g. Immunisation reminders & periodic review reminders. It may also be used for appointment reminders for a small number of clinics.
There is no cost to you the patient for text messages sent to your mobile from the surgery. If you choose to reply to a text message we send, then you will be charged the standard SMS cost depending on your network. Your information will not be disclosed to any third party and will be used by your healthcare professional only. If you would like to receive this service please fill in the attached form and return it to the Practice.
